Job Description
As a member of a multi-disciplinary team in a Program for Women and Children, you will provide a range of social work services based on family-centred care. This care includes perinatal loss; child protection concerns for the newborn, neonate and paediatric patient; crisis intervention counseling for parents and/or the obstetrical patient, and support to families facing birth anomalies, and acute and/or chronic illness.
Accountabilities:
- Advocate on behalf of the patient and liaise with the multi-disciplinary team and community agencies to access services and provide continued health care as identified
- Attend team meetings, consult with your professional peers and represent Osler as required
- Work closely with the other program social workers to cover the needs of all clients in the Women’s and Children’s Program
- As an integral part of the corporate Social Work team, participate in yearly strategic planning initiatives and corporate professional development
- You will report to the Manager of Women's and Children's Program, with professional accountability to the Professional Leader for Social Work.
- This role is integrated into the Paediatric Lifestyle Team, supporting Ontario Bariatric Peadiatric Network (OBPN), Intake and Activity Nights.
